General Instructions:
Please respond to the following questions in relation to your instructor and the class generally. On the first five questions, circle the number that corresponds to the following:

5 = Strongly agree; 4 = Agree; 3 = No opinion; 2 = Disagree; 1 = Strongly disagree

The instructor intends to use the results of this evaluation to improve future classes. Be as honest as you can, and add comments wherever they might be helpful. Your instructor will not receive the evaluations until after grades are submitted for the semester.
